Our dedicated team
We have a team of dedicated Clinical Nurse Specialists who work at Saint Catherine’s, who specialise in providing the community with palliative care.
Saint Catherine’s community specialist palliative care nurses work alongside patients and families enabling them to achieve their personal choices and discuss future care planning. The team provide specialist skills in helping to manage difficult physical, psychological, emotional and spiritual concerns.
We visit patients in nursing/residential homes and in patients’ homes around North and East Yorkshire.
We work closely with GPs and district nurses, consultants and multi-professional teams to ensure the patient care and support is well coordinated. We aim to complement the care these teams are already providing to manage patients’ symptoms and to enhance quality of life.
Working hours
The Team provides a 7-day service, including Bank Holidays, during office hours (8.30am-4.30pm), ensuring specialist palliative care is available to the community 365 days of the year. However they are not always available to take calls, so please leave a message with our administrator or on our confidential answerphone and we shall return your call as soon as possible:
We feel privileged to see patients in their own homes and work alongside patients and families to meet their individual wishes.
Referrals
We accept referrals from medical, nursing and therapy staff working in any care setting.
